ClergyMinisters conducting Chapel services must complete the "volunteer application".  Additionally, those conducting one-on-one visitations with inmates in the jail ministry booths must complete a "Ministerial Privilege Affidavit" as well as the "volunteer application".  Both are available on this site.  These forms may be brought or mailed to Chaplain Davis, SCDF, 141 West Elm St., Wichita, KS. 67203.
